Diabetes Management and Prevention using PEN Clinical Audit Tool
The PCS Clinical Audit Tool (CAT) is a data extraction tool for general practice that makes systematic diabetes prevention and management easier.
Diabetes Australia – Vic has produced this evidence-based resource to guide you through step by step instructions for using the CAT for effective prevention and management of diabetes.

New IDEAS for Diabetes Patients
A multidisciplinary team-based service for patients managing their diabetes is now available at the Whitehorse Community Health Service (WCHS). In partnership with Eastern Health, the Integrated Diabetes Assessment and Education Service - IDEAS - aims to support patients in preventing diabetes complications and better manage lifestyle factors impacting on their health.
A GP referral is required for an IDEAS consultation. GPs will be notified by fax when this referral is received at WCHS. The Endocrinologist will correspond with the referring GP and allied health staff and will send a faxed report following the patient's assessment.
The
Victorian Statewide Referral Form
identified as an IDEAS referral is prefered. Alternatively referrals can be faxed.
