General Practice Hospital Liaison program

The General Practice Hospital Liaison Program (GPL) has been established in conjunction with Eastern Health to improve communication between General Practitioners and Hospitals.

This program is provided by a team from three Divisions of General Practice (Melbourne East GP Network, Eastern Ranges GP Association and Greater Eastern Primary Health) and includes program staff and GP Consultants.

The GPL team endeavours to make sure that communication happens promptly and looks at the interface between General Practice and the Acute Sector. Multidisciplinary liaison meetings are held with each hospital and an overarching steering committee coordinates activities across all Eastern Health Sites. We also provide opportunities for GPs to improve their skills through a series of practical education forums.

Within the GPL team, Melbourne East GP Network has special responsiblity for Maternity Shared Care.
